There is one clinic in every city where a woman will drive past every other to reach.
Price never comes up. Distance never matters.
That chair is spoken for months in advance.
Every nurse in that city has quietly compared her own clinic to it, and none of them can explain it from the outside.
The same training.
The same machines.
The same techniques.
But nothing close to the same demand.
The answer has little to do with their quality of work.
It is their differentiation.
Most owners assume they have an attention problem.
So they run ads, and collect a handful of dead enquiries who were never going to come in, plus the usual price shoppers.
They decide advertising does not work for a clinic like theirs.
However, getting attention was never the problem.
Becoming the chosen was.
The clinics with waitlists earned them by leaving an IMPACT on a woman long before she ever enquired.

The ad is split into its three raw signals and each is run through a pretrained Meta model:
Seamless for audio, V-JEPA for video, Llama for text.
Each returns an embedding, a numerical description of what is actually in that second of footage, that sound, that sentence.
These models are used because their internal representations have been shown to line up with how
the human brain represents the same material.
For a clinic: the voice, the footage, and the words are measured separately, so the studio knows which of the three is working and which is dead weight.

2. Neural prediction.
The three embeddings feed a transformer that fuses them across time, learning representations that hold across every stimulus, task, and individual rather than fitting one person's quirks.
That is what makes the output general instead of anecdotal.
For a clinic: one person's opinion of the ad stops mattering. Not her husband's, not the front desk's, not the studio's. Only what holds across people.

3. Category mapping.
A final subject layer projects those representations onto 70,000 fMRI voxels across the whole brain.
A voxel is a 3D pixel of brain tissue, read indirectly through the slow changes in blood flow and oxygenation that follow neurons firing. The output is a predicted activity map, second by second.
For a clinic: a timeline of the ad with attention plotted along it. Second four holds, second nine drops. The drop is cut before a dollar finds it the slow way.
